The attached data collection forms are provided for the evaluation of SEAM ADDO program. This data collection effort will include visits to 50 ADDO facilities in Ruvuma and approximately 60 Duka la Dawa Baridi drug retail outlets in Singida. The same information will be gathered at each facility. The forms are designed to collect data on the indicators listed below, as well as a few additional supplementary indicators. These indicators are intended to measure the current status of the availability, affordability and quality of services, quality of key medicines at each of the facilities. In addition, five client satisfaction exit interviews will be conducted at each facility to determine their satisfaction with shop services.
